Scheduling Your Appointment

We will e-mail you to confirm your appointment time AFTER we have received your completed questionnaire and uploaded photos. Please select a day that is at least 24 hours or more after you plan to upload photos so that we have an opportunity to review them.

We will send you an e-mail confirming your appointment date and time after we have received your photos and questionnaire. Our meeting will take place via Zoom. To make the most of your consultation time, we strongly recommend downloading the Zoom app prior to our meeting. You can download Zoom’s video conferencing software here.

You can also click the link provided in our confirmation e-mail and enter the Meeting Number and Password to join. However, you may be prompted to sign in or create a Zoom account to participate so please do that prior to our consultation time.
